Pre-Registration

Welcome to pre-reg for Art of War II!

Individuals and Groups will use these same forms – please read these instructions carefully so we can get your registration confirmed error-free and quickly. Groups do not have to be family units, but do have to pay with a single check, money order, or ACCEPS transaction.

Instructions:
Pre-registration for this event ends on March 1, 2010. Registration will be confirmed via email; if you do not have email, please send a self-addressed, stamped envelope and we will mail your confirmation.

Individuals/group leaders must complete the Cover Page (page 2).

Registration section (pages 3-4): Fill out this page for all members of your group (please put the group leader at the top of the page). You may copy this page to add more people. Sub-total your registration fees and enter that amount on the Cover Page. Count the non-member adults in your group, and put that number on the Cover Page to calculate the total Non-Member Surcharges due. See the checklist on page 4 to insure you include copies of all appropriate documents (membership cards, waivers, etc).

Lodgings section (page 5): Please mark your 1st, 2nd and 3rd choices for lodgings, and list additional charges for those choices on the Cover Page. If you choose a no cost option then mark $0 in the lodging section. (This page includes notes about what type of bedding you should pack. Showers are available on site, so don’t forget your bath towel!)

T-shirt section (page 6): Please indicate your size and quantity selections and sub-total your t-shirt order. Put the total on the Cover Page, even if it is $0.

Double-check all your math on your cover page, before writing your check or obtaining your money order.

Use the check-list on the Cover Page to make sure you include all necessary documents before you seal your envelope! Also, we strongly recommend that you make a copy of all materials mailed, in case any questions arise, or things get lost in the mail.
